국내 문헌을 통한 의료기관 중심 가정간호서비스의 환자 성과
= A Review of Research on Hospital based Home Healthcare service Patient Outcome in Korea

초록[abstracts]
Hospital based home healthcare services have been regulated since 2001 and is still being used for the health care of patients discharged from the hospital to the home. Home healthcare nurses who provide home healthcare services are required high level qualifications and abilities because they need to visit their patients and provide their own therapeutic nursing services. It is also important to assess the patient's health outcomes for the services provided, since the home is an uncontrolled nursing environment unlike the hospital. This is related to the quality of service, and it is necessary to develop quality indicator based on patient’s outcome.
Therefore, this study attempted to search and analyze the documents measuring the patient outcomes of hospital based home care services among domestic papers published until August 31, 2017. A total of 2,475 papers were searched and 60 papers were used for the analysis, except for duplicate, not relevant to home healthcare, and secondary literature study papers. This papers were analyzed the Huffman’s five categories and types of patient outcomes: clinical outcome, functional outcome, medical outcome, financial outcome, and patient satisfaction.
The results of this study were as follows: 15 clinical outcome, 18 functional outcome, 12 medical outcome, 19 financial outcome, and 38 patient satisfactions outcome. The study of the patient outcome was most actively conducted (29 papers) in the period from the institutionalization of home care service in Korea to the implementation of long - term care insurance for the elderly (2001-2007).
The results of the analysis of the five categories of patient outcomes showed that the clinical outcomes were not significantly different between depression, pain, and anxiety among home care group and non – home care group, but depression and anxiety after maternal care is decreased. Functional outcomes showed no significant difference in the activities of daily living, self – care abilities, and musculoskeletal function between the home care group and the non – home care group. However, the functional status of surgical patients was improved after home care services. There was no difference in complication occurrence and hospital utilization rates in medical outcomes between home care group and hospital care group. In financial outcome, home care service has shown to be cost effective. In the patient satisfaction outcome, the average satisfaction rate of home care service was 85.4 (%) and the quality of life was 60.9.
This study is meaningful because it is analyzing patient outcomes of hospital based home healthcare service in Korea in various aspects for the first time. In order for the home healthcare service to be recognized as a good service for the patients and expanded in the future, there needs to be more research confirming the improvements in clinical outcome, functional outcome, and medical outcome associated with the service. Based on this study, it is necessary to establish a standardized quality management program for home health care services under government initiative.
초록[abstracts] 의료기관 중심 가정간호는 2001년부터 제도화되어 병원에서 가정으로 퇴원하는 환자들의 건강관리를 위해 이용되고 있다. 가정간호서비스를 제공하는 가정전문간호사는 환자 집을 방문하여 독자적으로 치료적 간호서비스를 제공해야 하므로 높은 수준의 자질과 능력이 요구된다. 또한 가정은 병원과는 달리 통제가 되지 않는 간호환경이므로 제공된 서비스에 대해 환자의 건강결과를 평가하는 것이 중요하다. 이는 서비스의 질과 관련되는 것으로 가정간호 환자 성과를 기반으로 한 질 지표의 개발이 필요하다. 따라서 본 연구는 국내 학술지에 2017년 8월 31일까지 게재된 논문 중 의료기관 중심 가정간호서비스의 환자성과를 측정한 문헌을 검색하여 분석하고자 시도되었다.
총 2,475편의 논문을 검색하였으나 이중 중복게재논문, 가정간호와 관련이 없는 논문, 2차 문헌 연구는 제외하여 총 60편의 논문이 분석에 이용되었다. 논문들은 Huffman(2004)이 제시한 임상적 성과, 기능적 성과, 의료적 성과, 재정적 성과 및 환자 만족도 성과, 총 5개의 환자 성과 범주와 유형을 이용하여 분석하였다.
본 연구 결과, 국내 의료기관 중심 가정간호 환자의 성과를 측정한 논문은 임상적 성과 15편, 기능적 성과 18편, 의료적 성과 12편, 재정적 성과 19편, 환자 만족도 성과 38편인 것으로 나타났다. 가정간호가 제도화된 이후부터 노인 장기요양제도 시행 전 (2001-2007년)까지가 국내 가정간호서비스가 활성화되는 시기로 환자 성과에 대한 논문도 29편으로 가장 활발히 이루어졌다.
5개 범주의 환자 성과를 측정한 논문을 분석한 결과, 임상적 성과는 가정간호군과 비가정간호군 간 우울, 통증, 불안 등에서 유의한 차이는 없었으나 산모에서는 가정간호 후 우울, 불안이 감소한 것으로 나타났다. 기능적 성과는 가정간호군과 비가정간호군 간 일상생활수행능력과 자가간호능력, 근골격 기능 등에서 대부분 유의한 차이가 없는 것으로 나타났으나 수술환자에서 가정간호 적용 후 기능상태의 호전을 보였다. 의료적 성과에서는 가정간호군과 병원간호군 간 합병증 발생과 의료기관 이용률에 차이가 없었다. 재정적 성과에서는 가정간호서비스가 비용절감의 효과가 있는 것으로 나타났다. 환자 만족도 성과에서는 가정간호서비스에 대한 만족도는 평균 85.4점(%), 삶의 질은 평균 60.9점으로 나타났다.
본 연구는 국내 의료기관 중심 가정간호서비스의 환자 성과를 다양한 측면에서 처음으로 분석한 것에 큰 의의가 있다고 본다. 향후 가정간호서비스가 대상자에게 좋은 서비스로 인식되고 더욱 확대되기 위해서는 임상적 성과, 기능적 성과, 의료적 성과를 입증하는 연구가 많아져야 한다. 또한 이를 기반으로 정부 주도하에 표준화된 가정간호서비스의 질 관리 프로그램을 마련이 필요하다.
