국민건강보험의 노인의료비 지출추계 및 장기재정 전망

초록[abstracts]
[본 연구의 목적은 65세 이상 노인의 인구변화 및 건강특성에 따른 건강보험의 장기재정지출을 전망함으로써 향후 인구고령화 진행에 따른 건강보험 재정에 대한 정책적 함의를 제시하는데 있다. 이를 위해 OECD요인별 추계방식을 사용하여 노인의 건강상태 변화, 노인인구 구조변화를 고려한 장기재정추계를 실시하였다. 분석결과, 첫째, 건강상태 변화(순수한 고령화, 건강한 고령화)를 고려하여 분석하였다. 순수한 고령화를 적용하여 추계한 결과, 2060년 노인의료비는 GDP대비 5.67%(시나리오1), 5.20%(시나리오2)로 나타났고, 건강한 고령화를 적용하여 추계한 결과, 4.97%(시나리오3), 4.50%(시나리오4)로 나타나 건강한 고령화의 의료비 절감효과가 확인되었다. 둘째, 건강한 고령화 상태에서 노인인구의 구조변화(고위인구, 중위인구, 저위인구)에 따른 재정추계를 실시하였다. 고위인구 가정시, GDP 대비 노인의료비는 최소 5.14%(고위 4)에서 GDP 대비 최대 6.39%(고위 1)로 나타났고, 저위 인구 가정시 2060년 기준 노인의료비는 GDP 대비 최소 3.93%(저위 4)에서 GDP 대비 최대 5.03%(저위 1)로 나타났다. 즉, 건강한 고령화 고려시, 2060년에는 순수한 고령화 대비 건강한 고령화의 절감효과는 평균적으로 GDP 대비 0.7%p에 달하는 것으로 나타났다. 이에 향후 노인인구 변화 및 건강상태 변화에 따른 변화를 볼 때, 국민건강보험의 지속가능성을 위해 인구구조를 고려한 건강한 고령화(healthy ageing) 실현을 위한 정책적 개입, 건강보험의 재정안정성 고려, 건강보험과 노인장기요양보험과의 관계 재정립 등이 필요하였다.
The purpose of this study is to propose policy implications for enhancing the sustainability of the system in the future due to the aging of the population by predicting the health status of 65 over aged and finances of national health insurance for the elderly. So, the method of analysis was based on the research method of OECD financial projections. The analytical method embodies the health status(pure ageing, healthy ageing), the population assumptions scenario(high, medium, and low) for each scenario and estimates financial outlooks for the beneficiary and expenditures of national health insurance by 2060. The results of the study were as follows: First, considering the health status, resulted showed that there are GDP of 5.67%(scenario1), 5.20%(scenario2) considering pure ageing, there are 4.97%(scenario 3), 4.50%(scenario4) considering healthy ageing in 2060. Second, considering the population assumptions scenario, resulted showed that there are GDP of min 5.14%(high4) - max 5.14%(high1) at the high population scenario in 2060. Resulted showed that there are GDP of min 3.93%(low4) - max 5.03%(low1) at the low population scenario. Thus, healthy ageing influences on reduction of health expenditures of the aged. These findings suggest that policy-makers must consider various conditions. In view of the changes in the recipients of the elderly population in the future, the government should consider policy measures such as diverse programs of healthy ageing, sustainability of national health insurance finances, rebuilding of national health insurance and long-term care insurance.]
