초록[abstracts]
최근 국내에서 시행되는 임상시험이 활발한 가운데, 항암제 임상 시험이 가장 많은 건수를 차지하고 있다. 면역세포를 활성화하여 암세포를 죽이는 면역항암제나 표적항암제 등에 대한 활발한 개발로 인하여 항암제 임상시험에서 필요로 하는 피험자 수는 계속적으로 증가하고 있다. 그러나 임상시험에 참여하는 암환자들은 밝혀지지 않은 약제의 안정성과 유효성 등으로 인하여 언제 발생할지도 모르는 부작용이나 중도탈락 등 불확실한 상황에 직면하게 되고 이는 결국 이들의 질병에 대한 적응을 어렵게 한다.
본 연구는 항암제 임상시험에 참여 중인 암환자의 불확실성, 대처, 그리고 적응 정도 및 관계를 파악하고, 적응에 영향을 미치는 요인을 확인하기 위한 서술적 상관관계 조사 연구이다. 대상자는 본 연구에 참여하기를 자발적으로 동의한 항암제 임상시험에 참여 중인 118명의 암환자이다. 자료는 2017년 7월 5일부터 8월 18일까지 설문지를 이용하여 수집하였다. 불확실성은 Mishel의 질병 불확실성 도구, 대처는 김종남의 한국판 암 대처 질문지, 그리고 적응은 Derogatis와 Lopez의 심리사회적 적응의 자가보고식 한국어판 도구를 사용 하였다. 자료는 SPSS 23.0 통계 분석 프로그램을 이용하여 기술통계, Pearson’s correlation coefficients, 단계적 다중회귀분석 방법으로 분석 하였다.
본 연구의 결과는 다음과 같다.
1) 대상자의 불확실성은 평균 41.34±11.42점, 대처는 평균 62.49±13.05점 으로 중등도 수준이었다. 대처의 하부영역인 대인간 대처의 문항 평균은 2.81±0.85점, 개인내 대처의 문항 평균은 2.66±0.53점 이었다. 또한 적응은 평균 158.24±24.28점으로 중등도 수준 이었다.
2) 대상자의 제 특성에 따른 적응은 연령(p=.038), 교육수준(p=.016), 경제상태(p=.014), 임상시험에 대한 만족도(p=.009), 임상시험에 대한 이해도(p=.040), 임상시험용 항암제 제형(p=.013), 임상시험 약으로 인한 불편감(p=.002)에 따라 유의한 차이가 있었다.
3) 대상자의 불확실성과 적응 간에는 통계적으로 유의한 음의 상관관계(r=-.46, p<.001)가 있었고, 대처와 적응 간에는 유의한 양의 상관관계(r=.50, p<.001)가 있는 것으로 나타났다.
4) 항암제 임상시험에 참여 중인 암환자의 적응에 영향을 미치는 요인은 대처, 불확실성, 임상시험용 항암제 제형 중 경구제와 주사제 병용, 연령, 임상시험에 대한 만족도 순으로 나타났다 (Adjusted R2=.45, p=.018).
결론적으로 항암제 임상시험에 참여 중인 암환자의 불확실성, 대처 및 적응 정도는 중등도 수준으로 나타났다. 불확실성, 대처 및 적응은 상호간에 상관관계가 있었으며, 적응에 유의한 영향을 미치는 요인은 대처, 불확실성, 임상시험용 항암제 제형 중 경구제와 주사제 병용, 연령, 임상시험에 대한 만족도 순이었다.
따라서 항암제 임상시험에 참여하는 암환자의 적응을 돕기 위해서는 대처 능력을 향상 시키고 불확실성을 낮추는 것이 필요하며, 특히 개인내 대처를 증진시키는 것이 필요하다고 본다. 동시에 적응에 영향을 미치는 제 요인들을 고려하여 임상시험에 참여하는 암환자들의 적응을 증진시킬 수 있는 맞춤형 중재 전략 개발이 필요할 것이다.

초록[abstracts]
The number of cancer patients participating in clinical trials of anticancer drugs are expected to be increased due to the active development of targeted therapy and immunotherapy. However, cancer patients in clinical trials are facing uncertain situations due to the unknown safety and efficacy of new drugs, and this uncertainty often produces psychosocial problems such as concerns about side effects, fear for dropping during the clinical trial process, and regret for giving up other treatments, and so on. The purpose of this study was to examine the relationships among uncertainty, coping, and adjustment in cancer patients who participate in cancer clinical trials and to identify factors influencing adjustment.
A descriptive correlation design was used. Before collecting the data, the study was reviewed and approved by the institutional review board to protect the rights of the participants. Data were collected from 118 cancer clinical trial participants from July 5 to August 18 in 2017. The instruments used in this study were the Mishel Uncertainty in Illness Scale, Korean-Cancer Coping Questionnaire, and Psychosocial Adjustment to Illness Scale. Data were analyzed using SPSS 23.0 program.
The findings were as follows:
1) The mean score of uncertainty in participants was 41.34±11.42 out of 80 and that of coping was 62.49±13.05 out of 92. The item mean of individual coping was 2.66±0.53 and that of interpersonal coping was 2.81±0.85. The average score of adjustment was 158.24±24.28 out of 230.
2) Adjustment was significantly different by age, education level, economic status, level of understanding of cancer clinical trials, satisfaction with cancer clinical trials, a type of an investigational product that is an anticancer drug, and physical discomfort due to an investigational product.
3) Adjustment of the participants was negatively correlated with uncertainty (r=-.46, p<.001) and positively correlated with coping (r=.50, p<.001).
4) Stepwise regression analysis revealed that the adjustment of the participants was explained by coping, uncertainty, a type of investigational products, age, and satisfaction with cancer clinical trials, accounting for 45.0% of the variance. The most influencing factor was coping.
In conclusion, the levels of uncertainty, coping, adjustment in cancer clinical trials participants were moderate, respectively. Significant correlations were identified among uncertainty, coping, and adjustment. Uncertainty and coping were significant predictors of adjustment. Therefore, interventions focused on decreasing uncertainty and improving coping with special attention to the individual coping strategies are needed to improve adjustment.
