조혈모세포 이식 혈액암 환자의 간호요구도

국문 초록[abstracts] 

본 연구는 D광역시 K대학병원에 내원하는 혈액암 환자들 중 조혈모세포 이식과정 전후의 환자들을 대상으로 상담한 상담일지의 내용을 분석하는 서술적 질적 연구이다. 2016년 1월 1일부터 2016년 12월 31일까지 상담한 환자 27명으로부터 도출된 상담자료 531건이 분석에 이용되었다. 연구결과, 정보적 제공요구, 신체적 증상관리요구, 심리사회경제적 지원요구로 분석되었다. 정보적 제공요구에는 조혈모세포이식전 정보제공과 이식후 정보제공으로, 신체적 증상관리요구에는 전신증상관리, 위장관계증상관리, 피부감각계증상관리, 근골격계증상관리, 신경계증상관리, 비뇨기계증상관리, 심혈관계증상관리, 호흡기계증상관리로, 심리사회경제적 지원요구로는 심리적 지원, 사회적 지원, 경제적 지원요구로 범주화되었다. 본 연구결과, 조혈모세포이식과정에서 혈액암환자들은 이식과정전후의 정보제공과 이식을 받고 난후 신체적 증상관리에 대한 요구가 두드러지게 나타났다. 본 연구결과를 토대로 혈액  암 환자의 간호요구를 이해하고 환자의 요구를 충족시키는 상담 프로토콜의 개발과 통합적 간호중재 프로그램의 개발을 위한 기초자료로 이용될 수 있을 것이다.

다국어 초록[Multilingual Abstract]

The present study is descriptive qualitative research that analyzed the contents of counseling journals written during counseling the patients before and after the process of stem cell transplantation among hematologic malignancy patients who visited K University Hospital in D Metropolitan City. A total of 531 counseling data derived from 27 patients counseled from January 1 to December 31, 2016 were used in the analysis. The results showed that subjects  demands were the provision of information, management of physical symptoms, and psycho-socio-economic support. Demand for information provision was categorized into the provision of information before and after stem cell transplantation. Demand for physical symptom management was categorized into the managements of systemic general symptoms, gastrointestinal tract  systems symptoms, sensimeter symptoms, musculoskeletal system symptoms, neurological system symptoms, urinary system symptoms, cardiovascular system symptoms, and respiratory system symptoms. Demand for psycho-socio-economic support was categorized into psychological support, social support, and financial support. The results of the present study showed that the demand of patients with hematologic malignancy for the provision of information before and after stem cell transplantation and the demand for the management of physical symptoms after transplantation were statistically significant. The results of the present study can be used as basic data for understanding the nursing demands of patients with hematologic malignancy, the development of counseling protocols that satisfy the demands of patients, and the  development of integrated nursing intervention programs.
