초록[abstracts]
Purpose : Ethical sensitivity is the essential competency for nurses to practice ethical nursing care. The purpose of this study was to develop and test validity and reliability of the ethical sensitivity scale for clinical nurses.
Method : This is a methodological study that aimed to develop and validated the ethical sensitivity scale. Based on the conceptual framework, 55 preliminary items was generated by literature review, existing scales and in-depth interviews. As a result of two rounds of content validity, 40 items were derived. The sacle validation was conducted with a convenience sample of 344 general hospital nurses recruited from four general hospitals in Busan and Gyeongnam area.
Result : For construct validity, we used item analysis, factor analysis, correlation between total points and factor. In accordance with classical testing theory, item analysis was conducted. A total of 3 items were deleted through the item analysis. After the exploratory factor analysis for 37 items, 8 factors with 36 items were retrieved for 61.30% of the total variance. The factors identified were labeled patient respect, professional ethics, nursing practice responsibilities, empathy, perception of ethical situation, ethical burden, ethical contemplation, and willingness to do good.
In order to verify the convergent/discriminant validity, a Multitrait/Multi-item Matrix analysis was performed, and all the items showed a high correlation with their subregions and a low correlation to distinguish them from the non-relevant regions. The result of correlation with Korean Moral Sensitivity Questionnair (Han et al., 2010) for criterion validity showed the correlation coefficient was .52(p<.001), which proved the validity of the developed scale. The result of the group comparison showed that there were significant differences according to clinical career and working department. The reliability was tested by internal consistency using Cronbach's α (.92), a split-half reliability test (.94) and a test-retest (.89) to evaluate the homogeneity and stability of the scale. The last revised theoretical model consisted of the final 34 items showed all goodness-of-fit parameters (χ2/df=1.80, GFI=.86, RMSEA=.05, NFI=.81, NNFI=.90, CFI=.91) after three round confirmatory factor analysis for the 36 items resulting from EFA. As a result of the reliability test for the internal consistency of the 34 items, Cronbach's α coefficient was .92.
Conclusion : The scale that developed to measure clinical nurses' ethical sensitivity in this study was proved to have validity and reliability. In addition, this scale is able to contribute to developing the strategies to improve nurses' ethical sensitivity in nursing settings and the educational program to enhance nursing competency related to ethical sensitivity in nursing college.
