‘호스피스 환자 영적 고통 측정 간이 도구’의 구성 타당도 중심의 검증

Spiritual care is a principal component of palliative care. Assessment of spiritual state of patients is the first step in spiritual care. There are few tool evaluating spiritual state of Korean terminally ill cancer patients. A 'brief scale assessing spiritual suffering for hospice patients' was developed by multi-center study based on conceptualization of spirituality in Korean terminally ill cancer patients. Thus, this study aimed to validate a 'brief scale assessing spiritual suffering for hospice patients' focusing on construct validity. Among the patients who admitted to the hospice-palliative care unit of Chungnam National University Hospital in Daejeon from March 2015 to September 2015, 40 patients were enrolled. Sociodemographic characteristics (age, sex, educational state, monthly income, marital status, religion, activities of daily living), a 'brief scale assessing spiritual suffering for hospice patients', FACIT-SP 12 (Functional assessment of chronic illness therapy-spirituality 12), and EORTC QLQ-C15-PAL (European organization for research and treatment of cancer quality of life questionnaire-core 15-palliative care) were investigated by self-administered questionnaire. I helped patients to answer properly and checked performance status of patients using PPS (Palliative performance scale). Internal consistency and split-half coefficients were calculated to analyse reliability of a 'brief scale assessing spiritual suffering for hospice patients'. Factor analysis was performed to analyse construct validity. Spearman's correlation analyses were conducted to investigate correlation among 'brief scale assessing spiritual suffering for hospice patients', FACIT-SP 12, EORTC QLQ-C15-PAL, and PPS. The following results were obtained. Subjects who are divorced, whose religion is protestants, and who have hepatobiliary/gallbladder cancer showed relatively low spiritual suffering. Cronbach's α, Spearman-Brown split-half coefficient, and Guttman split-half coefficient of a 'brief scale assessing spiritual suffering for hospice patients' were 0.836, 0.861, and 0.857, respectively. Four factors were extracted by factor analysis and total of explained variance was 82.01%. I suggest 'looking back one's life', 'consolation of life', 'completion of life', and 'self-esteem' as four factors consisted in spiritual suffering. Total score of a 'brief scale assessing spiritual suffering for hospice patients' was negatively correlated with spiritual well-being measured by 2 factors (meaning/peace and faith) of the FACIT-SP 12 and quality of life measured by the EORTC QLQ-C15-PAL. Total score of a 'brief scale assessing spiritual suffering for hospice patients' was positively correlated with emotional functioning (depression, anxiety), dyspnea and anorexia measured by the EORTC QLQ-C15-PAL. Total score of 'brief scale assessing spiritual suffering for hospice patients' was not significantly correlated with the PPS. Above results suggest that a 'brief scale assessing spiritual suffering for hospice patients' is reliable and valid tool evaluating spiritual state of terminally ill Korean cancer patients.
